La storia inizia nel 1919 nella povertà della Birmingham del primo dopoguerra dove tutti lottano per sopravvivere ad un periodo particolarmente difficile dal punto di vista storico ed economico. La storia è ambientata nel quartiere di Small Heath e si focalizza particolarmente sulla famiglia Shelby, il cui capofamiglia è anche il boss della gang detta "Peaky Blinders", dall'usanza di nascondere una lametta nel risvolto dei cappelli, in modo tale da poterla utilizzare anche come arma.
La leadership di Tommy Shelby è messa a dura prova dall'arrivo in città da Belfast dello spietato poliziotto Campbell, deciso a ripulire il marcio e la delinquenza a tutti i costi, e da una donna, Grace Burgess, dal passato misterioso.
